No, NOT a trick, a Halloween treat! Presenting the Dead of the Brain 1 English patch by David Shadoff for the DEAD last official PC Engine CD game published by NEC before exiting the console biz in 1999! I actually used my knowledge of Rastan 1 to come up with that answer since it has similar icons.  But I know that the PC Engine is nowhere near powerful enough to handle Rastan 1, only the mighty Sega Master System would even dare accomplish that task.... so it just had to be Rastan Saga 2.

Anyway here's the next one:
